Job summary

CircleLink Health is hiring 10 elite nurses for a fully remote, 1099 contractor role. You’ll manage a large Medicare patient panel, delivering monthly clinical calls to drive outcomes and reduce hospitalizations.

We value autonomy, efficiency, and performance — with income reflecting your output. This role offers flexible scheduling between 8am-5pm EST and a clear path to higher earnings as you hit high-performance benchmarks.

Qualifications

• Compact RN license and 3+ years clinical experience.
• Extremely self-directed with ability to manage large caseloads (>100 patients/month).
• Strong organizational skills, metrics-driven and tech fluent.
• Comfortable working with multiple practices, workflows and EHRs.
• Excellent communication and critical thinking.

Responsibilities

• Run monthly clinical calls with chronic care patients.
• Drive behavior change through coaching and education.
• Maintain precise, compliant documentation in the platform.
• Close preventive care gaps and coordinate services.
• Update care plans and track interventions.
• Support transitions of care to reduce readmissions.
• Manage your panel efficiently and consistently.
