CircleLink Health is hiring elite nurses for a fully remote, 1099 contractor role. You'll manage a large Medicare patient panel, delivering monthly calls to drive outcomes and reduce hospitalizations. The role rewards performance with scalable earnings and no cap on productivity.

Qualifications

• Compact RN licensed and active.
• 3+ years clinical experience.
• Strong internet and dedicated workspace.
• Confident with multiple software platforms.
• Excellent communication and critical thinking.

Responsibilities

• Run monthly clinical calls with chronic care patients.
• Drive behavior change through coaching and education.
• Maintain precise, compliant documentation using our platform.
• Close preventive care gaps and coordinate services.
• Update care plans and track interventions.
• Support transitions of care to reduce readmissions.
• Manage your panel efficiently and consistently.
