Job summary

Urban Health Plan seeks a Remote Registered Nurse for the Virtual Care Center to triage acute cases, prioritize patient flow, and support telemedicine visits. The role requires a NYS RN license, 1-2 years in primary care, and English/Spanish bilingual abilities.

The position is remote with the option to come onsite in Bronx locations as needed, and includes comprehensive benefits with a strong focus on patient-centered telehealth care.

Qualifications

• Bachelor's Degree in Nursing is required.
• NYS Licensed Registered Professional Nurse.
• 1-2 years of direct clinical experience in a primary care environment working with adults and children.
• Bilingual English/Spanish required.
• Experience using electronic health records.
• Ability to thrive in a telemedicine environment.

Responsibilities

• Triages patients based on acuity and ensures timely visits.
• Monitors call center template for pediatric and adult patients and discharged patients.
• Manages abnormal labs/DI and critical values.
• Answers clinical calls from hospitals, pharmacies, DOH, and call center agents.
• Follows up with RPM patients to assess acuity based on vitals.
• Assists Virtual Care Center Medical Assistants with clinical guidance and support.
• Directs patient flow to providers based on acuity.
