Job summary

Firefly Health is seeking a Registered Nurse to join its remote care team supporting patients in Massachusetts. The RN will triage new concerns via the app or phone, coordinate appropriate levels of care with clinicians, and provide patient-centered education and follow-up for acute and chronic conditions.

Responsibilities include prescription management coordination and collaboration with a multi-disciplinary team to support patients with hypertension, diabetes, and anticoagulation needs.

Qualifications

• Bi-lingual English/Spanish with medical terminology.
• Massachusetts RN license is required.
• Experience with EMR systems and telehealth is preferred.

Responsibilities

• Triage acute and chronic conditions via Firefly app or calls and coordinate care with clinicians.
• Provide patient education on disease management and follow-up for evolving illnesses.
• Coordinate care across settings, including post-ED and discharge planning.
• Deliver culturally sensitive, patient-centered care aligned with member-first principles.
• Respond to patient messages and calls during shift hours.
