Connect America is seeking a Remote Care Nurse II to deliver high-quality nursing care to patients in a remote setting via telehealth. You will perform remote assessments, develop care plans, and coordinate with a multidisciplinary team to improve outcomes.

The role requires an active US RN license, experience in telehealth or case management, and strong communication skills to support patients and families across diverse populations.

Qualifications

• Active RN license in the United States.
• Compact state license with CA/IL preferred.
• Minimum of two years clinical nursing experience.
• Proficiency with electronic health record (EHR) systems and telehealth technology platforms.
• Strong communication skills to effectively engage with patients and teams remotely.
• Ability to work independently in a remote environment while managing multiple patient cases.

Responsabilités

• Conduct remote patient assessments via phone or video to evaluate health status and care needs.
• Develop, implement, and update individualized care plans with patients and care teams.
• Monitor patient progress and adherence, providing education for self-management.
• Coordinate with physicians, specialists, and other professionals for seamless care delivery.
• Document all patient interactions and findings in EHRs per regulatory standards.
• Respond promptly to patient inquiries or urgent concerns and escalate as needed.
• Participate in quality improvement and continuing education to maintain competencies.
