Job summary

BOUNDLESS HOME HEALTH LLC seeks a dedicated Registered Nurse to create and manage individualized care plans for patients. You will observe, document, and communicate changes to ensure the patient’s best interests are served with attention to detail and compassion.

Ideal candidates hold an active RN license, strong interpersonal and problem-solving skills, and the ability to drive care through accurate charting and collaboration with the care team.

Qualifications

• Currently licensed Registered Nurse (RN) in state of employment
• Previous RN experience preferred
• Excellent interpersonal and client care skills
• Valid driver’s license
• First aid/CPR certification required

Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
• Administer medications and monitor patients for side effects or reactions
• Document any changes in patient conditions and report on changes
• Maintain accurate records of patient care and discuss observations with the supervisor
