Job summary

Boundless Home Health is seeking a Registered Nurse to join our team as a remote caregiver. You will create and manage patient care plans, observe and communicate effectively, and apply up-to-date nursing practices with compassion.

The role requires an active RN license, CPR/First Aid, a valid driver's license, and strong interpersonal skills. You will coordinate care with colleagues, administer medications, document changes, and maintain accurate patient records.

Qualifications

• Current RN license in the state of employment.
• Prior RN experience preferred.
• First aid/CPR certification required.

Responsibilities

• Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans.
• Administer medications and monitor patients for side effects or reactions.
• Document any changes in patient conditions and report on changes.
• Maintain accurate records of patient care and discuss observations with the supervisor.
