Job summary

Sylvan Health is seeking a mission-driven Registered Dietitian to provide telehealth nutrition therapy, deliver individualized care, and collaborate with a cross-functional care team. You will implement evidence-based clinical programs, monitor outcomes, and support patients in making sustainable lifestyle changes.

Ideal candidates hold IL licensure, a Bachelor's in Nutrition/Dietetics, and an accredited internship with CDR credentials, plus 1–2 years of post-internship experience counseling

Qualifications

• Licensure in IL strongly preferred
• Bachelor's Degree in Nutrition/Dietetics
• Completion of Accredited Dietetic Internship
• Credentialed by the Commission on Dietetic Registration (CDR)
• Minimum 1-2 years of post-internship nutrition experience
• Experience counseling patients virtually or in an outpatient setting
• Expertise in GI conditions (IBD, IBS, fatty liver, celiac, etc.)
• Comfort educating patients with chronic conditions including diabetes and kidney disease

Responsibilities

• Conduct personalized nutrition therapy consultations via telehealth
• Administer evidence-based clinical protocols and provide highly individualized nutrition support
• Monitor patient progress and adjust plans to improve outcomes
• Achieve high patient retention by following program guidelines
• Collaborate with care team members across disciplines
• Leverage meal planning tools to aid patient adherence
• Participate in clinical rounds with partner practices and care teams
• Attend RD group meetings and monthly 1:1s with CNM
• Support patients in making lifestyle changes
• Develop trust quickly with patients and caregivers
• Document clinical information in an Electronic Medical Record
• Work in a fast-paced, evolving environment with a positive attitude
• Transition between patients efficiently and follow clinical procedures

Education

Bachelor's Degree in Nutrition/Dietetics

Accredited Dietetic Internship

CDR Credentialed

Licensure in IL strongly preferred

1-2 years of post-internship nutrition experience

Experience counseling patients virtually/outpatient

GI conditions expertise (IBD, IBS, fatty liver, celiac)

Diabetes and kidney disease patient education
