Job summary

Sylvan Health is seeking a mission-driven dietitian to provide telehealth nutrition therapy and evidence-based care. You will administer clinical programs, monitor progress, and collaborate with a multi-disciplinary care team to optimize patient outcomes.

Ideal candidates have a bachelor's in Nutrition/Dietetics, completed accredited internship, and CDR credentialing, with 1–2 years of post-internship experience and comfort counseling patients remotely.

Qualifications

• Bachelor's Degree in Nutrition/Dietetics is required.
• Completion of Accredited Dietetic Internship is required.
• Credentialed by the Commission on Dietetic Registration (CDR) is required.
• Minimum 1–2 years of post-internship nutrition experience.
• Experience counseling patients virtually or in outpatient settings is preferred.

Responsibilities

• Conduct personalized nutrition therapy consultations via telehealth.
• Administer evidence-based clinical protocols and deliver individualized nutrition support.
• Monitor patient progress and adjust plans to improve outcomes.
• Collaborate with care team members to support patient care.
• Document clinical information in an Electronic Medical Record.
