Holon Health is seeking a mission-aligned Recovery Coach to bring the power of lived experience to the center of our care model. As a certified Peer Recovery Support Specialist, you’ll support justice-involved individuals on their recovery journey—offering encouragement, building trust, and helping navigate barriers that stand between them and lasting wellness.

Qualifications

• Sustained recovery with at least one year of lived experience (strongly preferred).
• PRS certification or equivalent required.
• Empathetic with strong listening and communication skills.
• Knowledge of recovery barriers and justice-involved populations a plus.

Responsibilities

• Build trusting relationships with clients using shared lived experience.
• Offer hope, motivation, and accountability from personal recovery.
• Collaborate with the care team to develop personalized recovery plans.
• Connect clients to housing, healthcare, employment, and transportation resources.
• Conduct strengths-based assessments to identify needs and goals.
• Advocate for clients within the justice and care systems.
• Partner with Care Navigators and clinicians for integrated care.
• Stay informed on best practices in peer support and recovery-based care.
• Maintain HIPAA confidentiality and compliance.
• Contribute to new initiatives and engagement strategies.
