Job summary

Holon Health is seeking a mission-aligned Recovery Coach (PRS) to place lived experience at the center of our care model. This full-time, remote role supports justice-involved individuals on their recovery journey, offering encouragement and helping navigate barriers to lasting wellness.

As a certified PRS/Peer Recovery Coach, you will work with the care team to develop personalized recovery plans, connect clients to housing, healthcare and employment resources, and uphold HIPAA confidentiality.

Qualifications

• Sustained recovery with at least one year of lived experience (preferred).
• PRS or Peer Recovery Coach certification required.
• Strong listening and communication skills.
• Remote, fast-paced, team-based environment.
• Aligned with Holon’s values of equity and inclusion.

Responsibilities

• Build trusting, nonjudgmental relationships with clients based on shared lived experience.
• Use personal recovery experience to offer hope, motivation, and accountability.
• Collaborate with the care team to develop personalized recovery plans.
• Connect clients to housing, healthcare, employment, and transportation resources.
• Conduct strengths-based assessments to identify client needs and goals.
• Advocate for clients within the justice system and care system.
