Qualifications

Responsibilities

• Coordinate and oversee the Family Mentor Program, including recruitment and training of mentors.

• Monitor outcomes and maintain case documentation.
• Collaborate with community partners and interdisciplinary teams.
• Facilitate mentor support groups and outreach efforts.
