Job summary

PanCare of Florida is seeking a Registered Dietitian Nutritionist (RDN) to provide nutritional assessments, MNT, and nutrition education for patients via telehealth and in-person visits. The role supports a HRSA Telehealth Nutrition Services Network Grant Program and works with a Community Health Worker to connect nutrition care to primary care and behavioral health.

The RDN will deliver remote patient monitoring, lead group visits, and develop nutrition programming, including diabetes

Qualifications

• Bachelor’s degree in dietetics or nutrition completed.
• RDN credential from CDR.
• Experience providing MNT and nutrition counseling.
• Ability to pass background and drug screening.

Responsibilities

• Coordinate telehealth visits and ensure nutrition recommendations reach care teams.
• Perform comprehensive nutritional assessments for referrals.
• Provide MNT and counseling per Florida licensure.
• Deliver telehealth visits and in-person as needed.
• Utilize RPM and telehealth tools to monitor progress.
• Lead group nutrition visits and diabetes education.
• Lead cooking demonstrations and meal-prep instruction series.
