Zusammenfassung

PanCare of Florida is seeking a Registered Dietitian Nutritionist (RDN) to provide nutritional assessments, Medical Nutrition Therapy (MNT), and education for patients with or at risk of diet-related chronic conditions.

The RDN will deliver nutrition services via telehealth and in person, collaborating with a Community Health Worker across primary care, behavioral health, and prenatal services.) The role is home-based with site visits as needed, supporting EHR integration, UDS reporting, and

Qualifikationen

• Bachelor’s degree in dietetics or nutrition from ACEND-accredited program.
• RDN credential from the Commission on Dietetic Registration.
• Experience providing MNT and nutrition counseling for diet-related chronic conditions.
• Ability to pass background and drug screening.

Aufgaben

• Conduct comprehensive nutritional assessments for referred patients (diabetes, hypertension, hyperlipidemia, cardiovascular disease, overweight, obesity).
• Provide MNT and individualized nutrition counseling per evidence-based guidelines.
• Deliver telehealth and in-person visits; use RPM and telehealth platforms.
• Lead group nutrition visits and cooking demonstrations; coordinate program activities.
• Support onboarding of originating sites and MOA information; ensure referrals are integrated into care.
