Job summary

Net Health (Limber Health) is seeking a Care Navigator to support patients through home-exercise programs, monitor progress remotely, and collaborate with clinicians for improved musculoskeletal outcomes.

The ideal candidate will have a Minnesota PTA license, 2+ years in musculoskeletal health, and strong skills in empathy, communication, and data-driven decision-making. This role offers remote work with a competitive hourly wage.

Qualifications

• Must have graduated from an accredited physical therapy program.
• Active PTA license in Minnesota.
• 2+ years of experience in musculoskeletal health is a plus.
• Tech-savvy; able to navigate multiple platforms (EHR, messaging, scheduling).
• Excellent communication, interpersonal, and data presentation skills.

Responsibilities

• Provide remote monitoring to support patients in home-exercise therapy.
• Direct and assist users in therapeutic exercise and functional training through the platform.
• Provide feedback to clinicians on patient progress outside the clinic.
• Maintain records and notes while remote monitoring.
• Assist with improving platform processes and functionality.
• Organize and manage a caseload efficiently.
