Job summary

Net Health, via Limber Health, is recruiting a Care Navigator to support patient success through home-exercise therapy and remote monitoring. You will help patients engage in therapeutic exercises, foster trust, and collaborate with clinicians to guide interventions and improve outcomes.

Ideal candidates are certified PTAs with an active Illinois license, strong communication skills, and comfort with multiple health IT platforms.

Qualifications

• PTA with active Illinois license and graduation from an accredited physical therapy program
• Experience with musculoskeletal health is a plus (2+ years)
• Tech-savvy with multi-platform experience (EHR, scheduling)

Responsibilities

• Provide remote monitoring services to support patients in home-exercise therapy.
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through our technology platform.
• Provide ongoing feedback to the evaluating clinician on patient progress outside of the clinic.
• Maintain records and notes while improving platform processes and functionality.
• Organize and manage a caseload efficiently.
