Job summary

Limber Health, a business unit of Net Health, seeks a Care Navigator to support patients remotely in musculoskeletal health through home-exercise therapy and telehealth workflows. You will work across EHR, scheduling, and messaging platforms to guide interventions and track progress.

The ideal candidate is an empathetic, independent, tech-savvy PTA with 2+ years in musculoskeletal health, comfortable engaging diverse users, and able to thrive in a fast-growing startup environment focused on

Qualifications

2+ years of experience in musculoskeletal health.
Graduated from an accredited physical therapy program and hold an active PTA license in Minnesota.
Technically proficient and comfortable with multiple platforms (EHR, scheduling, messaging).
Excellent communication, interpersonal, and data presentation skills.

Responsibilities

Provide remote monitoring to support patients in home-exercise therapy.
Direct and assist users in therapeutic exercise and functional training via our platform.
Provide ongoing feedback to clinicians on patient progress outside the clinic.
Maintain records and notes while delivering remote monitoring.
Assist with improving platform processes and functionality.
Organize and manage a caseload efficiently.
