Net Health is seeking a Minnesota-licensed Physical Therapy Assistant (Care Navigator) for a remote role focused on musculoskeletal health. You will support patients through home exercise programs, perform remote monitoring, and help clinicians track progress and outcomes.

The ideal candidate has an accredited PTA program, an active Minnesota license, and comfort navigating multiple EHR and scheduling platforms.

Qualifications

• Graduated from an accredited physical therapy program.
• Active Minnesota PTA license.
• 2+ years in musculoskeletal health is a plus.
• Experience with remote patient monitoring and home exercise programs.
• Strong data collection and reporting abilities.

Responsibilities

• Provide remote monitoring for home-exercise therapy.
• Guide users through therapeutic exercise and functional training.
• Provide feedback to clinicians on patient progress.
• Maintain records and notes for remote monitoring.
• Organize and manage a caseload efficiently.
