Job summary

TeamHealth is seeking a quality-driven nurse practitioner to join our remote telephonic post-acute care call coverage team for weekend day coverage. Full-time and part-time opportunities are available, offering autonomy and strong support.

You will provide high-quality telephonic care to a diverse patient population, collaborating with an experienced multidisciplinary team and benefiting from onboarding programs and ongoing clinical guidance.

Qualifications

• NP license in good standing (multi-state preferred).
• Experience in post-acute care or telephone triage preferred.
• Strong communication and team-collaboration skills.

Responsibilities

• Triage and adjust personalized care plans.
• Coordinate care to improve outcomes.
• Collaborate with nursing staff, physicians and other professionals.
• Maintain accurate documentation per regulations.
