Qualifications

• RN or LPN license required or Maine-specific SAMHSA X-DEA training
• Bachelor's degree in Nursing preferred
• At least 5 years direct patient-care in addiction medicine or behavioral health

Responsibilities

• Maintain a registry of members in dedicated special populations
• Collaborate with care team to coordinate treatment plans
• Provide direct member care via telephonic and telemedicine encounters
• Support pilot projects and quality improvement initiatives
• Assist in training staff on the Collaborative Care Model
