Job summary

Groups Recover Together is seeking a Population Health Coordinator to support delivery of high‑quality care to select high‑acuity member populations and during transitions in their recovery journey. The role centers on data management, care coordination, and direct member care within a Collaborative Care Model.

The position requires RN or LPN credentials with SAMHSA X‑DEA training and at least 5 years of direct patient care in addiction or related behavioral health.

Qualifications

• RN or LPN with SAMHSA X‑DEA training required
• Bachelor's degree in Nursing preferred
• At least 5 years direct patient‑care experience in addiction medicine or behavioral health
• Ability to work remote with reliable internet and strong tech setup

Responsibilities

• Support registry review and data management within Collaborative Care Model
• Assist in care coordination across local care teams and document outcomes
• Provide direct member care via telephonic and telemedicine encounters
• Attend interdisciplinary meetings and escalate urgent situations as needed
