Vigilance Health invites a qualified Quality Improvement Project Manager to bridge Vigilance with its ACO and population health partners. You will lead stratification, measure performance, and drive improvement projects that close gaps in care and boost quality scores.

You will work closely with partner QI teams and Vigilance leadership, translating data insights into actionable interventions and expanding patient panels across our network.

Qualifikationen

• RN license is required and must be current.
• Subject matter expert in ACO quality measures and population health quality improvement.
• Experience with Johns Hopkins ACG or similar risk stratification platform.
• Proven ability to build trust and deliver results with CMOs and clinical leaders.
• Strong data instincts and ability to tell a story from data.
• 5+ years of clinical experience in population health, quality improvement, or ACO operations.

Aufgaben

• Build and maintain relationships with partner QI teams and Vigilance-facing stakeholders.
• Deep dive into ACG and Vigilance Care Portal to understand data, tech, and reporting.
• Generate and maintain baseline reports on ACO quality measures for partners and Vigilance.
• Develop data-driven strategies and translate them into workflows, trainings, and interventions.
• Identify opportunities for new services, renewals, and expanding patient panels.
