Privia Health is seeking a Population Health Associate to support population health initiatives for an attributed patient population. The role partners with Quality, Risk Adjustment, Population Health Leadership, and Practice Operations to develop action plans and improve quality performance, coding, and clinical documentation in value-based care.

The position involves both virtual and in-person engagement with Privia providers and care centers, travel to meetings, and occasional after-hours

Competenze

• Bachelor's degree required; healthcare experience preferred.
• 3+ years in a medical/healthcare setting.
• Experience with value-based care programs (MSSP, MIPS, HEDIS or STARS) preferred.
• Fluent in EMR clinical workflows; athenaNet experience preferred.
• Basic knowledge of coding and documentation, especially HC coding.
• Strong Excel skills.
• Must reside in the market of assignment; HIPAA compliance.

Mansioni

• Review and analyze performance data from payers and Privia analytics to create actionable reports with measurable goals for access, quality, coding, and documentation.
• Present data and reports to providers and care center staff during on-site or virtual meetings.
• Maintain ownership of performance metrics for assigned care centers.
• Build collaborative relationships to drive Value-Based Care program performance.
• Provide quality measure training and other presentations to staff and providers.
• Offer workflow guidance and troubleshooting based on EMR knowledge to support value-based care.
• Analyze medical records to identify quality measure gap closures and trends.
• Submit supplemental data to payer portals.
• Participate in special projects as needed.
