Job summary

Vantage Health Partners seeks a Physical Therapist to join our Home Health/Assisted Living Facilities team covering Loveland and Fort Collins, CO. This remote-based clinical role provides rehabilitative care to help patients regain mobility and independence.

You will evaluate patients, develop individualized plans, and guide therapeutic exercises while collaborating with physicians and nurses. Compensation includes pay-per-visit or salaried options, with details shared during the hiring process.

Qualifications

• DPT degree from an accredited program.
• Current Colorado licensure or eligibility.
• Strong knowledge of musculoskeletal, neurological, and cardiopulmonary rehab.
• Proficiency in patient assessment, documentation, and clinical reasoning.
• Excellent interpersonal and communication skills.
• Ability to work in a fast-paced, on-site healthcare environment.
• Commitment to patient safety, empathy, and quality of care.
• Preferred: EMR experience and outpatient or hospital-based rehabilitation.

Responsibilities

• Evaluate patients' physical condition and functional limitations.
• Develop and implement individualized physical therapy treatment plans.
• Perform therapeutic exercises, manual therapy, and rehabilitation techniques.
• Educate patients and families on injury prevention and home exercise programs.
• Monitor and document patient progress and treatment outcomes accurately.
• Adjust treatment plans based on patient response and clinical findings.
• Collaborate with physicians and interdisciplinary care teams.
• Maintain compliance with clinical protocols, safety standards, and regulatory requirements.
• Provide clear communication and compassionate care to patients and their families.
• Participate in continuing education to stay current with best practices in physical therapy.
