Job summary

Accounting Search is seeking a dedicated Physical Therapist for home health care in Colorado. This full-time role focuses on delivering patient-centered therapy in patients' homes, improving mobility and independence.

You will perform assessments, design individualized treatment plans, guide exercises, educate families, document progress, and collaborate with a multidisciplinary team while ensuring regulatory compliance.

Qualifications

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience
• Must possess and maintain CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations

Responsibilities

• Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately.
• Create and implement tailored treatment plans that address rehabilitation goals and patient needs.
• Conduct therapeutic exercises and training programs to improve mobility and functionality.
• Educate patients and families about injury prevention, wellness strategies, and the recovery journey.
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve rehabilitation goals.
• Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.
• Maintain meticulous patient records and documentation, complying with regulatory standards.
