Job summary

Qualifications

• Licensed Clinical Social Worker (LCSW) in NY required.
• Master's Degree in Social Work required.
• Minimum of four years direct clinical social work experience in healthcare setting.

Responsibilities

• Collaborates with the transitional care team to perform interventions after hospital discharge.
• Make home visits to patients and develop care plans with the team.
• Conduct weekly or more frequent phone visits for assessment and support.
• Provide psychosocial services, crisis intervention, and community resource planning.
• Documentation and reporting in electronic medical records for continuity of care.
• Participate in discharge planning and program evaluation.
