CVS Health in New York (remote) is hiring an Analyst, Care Manager to provide comprehensive care management to New York MLTC members. You will conduct psychosocial assessments, develop person-centered service plans, and coordinate long-term services, supports, and resources.

The role requires an MSW and active NY LMSW or LCSW licensure, plus 2+ years in care management. Strong communication and collaboration with interdisciplinary teams are essential, with travel as needed within the service

Qualifications

• MSW from an accredited program and NY licensure required or preferred.
• Minimum 2 years in care management or related healthcare fields.
• Knowledge of NY Medicaid/Medicare MLTC and LTSS helpful.

Responsibilities

• Conduct psychosocial assessments for MLTC members.
• Develop and implement person-centered care plans.
