Job summary

Vaya Health is seeking a Peer Support Specialist to conduct in-reach and engagement activities with adults who have serious mental illness. You will educate members about community-based services, tenancy supports, and rental assistance while maintaining HIPAA and confidentiality obligations.

The role includes coordinating face-to-face or remote meetings, supporting transitions to housing, and ensuring outreach aligns with NC State Plan for Medical Assistance where applicable.

Qualifications

• High school diploma or GED is required for this position.
• NC Certified Peer Support Specialist certification is required.
• Experience working with MH/SU populations is preferred.
• Strong communication and interpersonal skills are essential.

Responsibilities

• Perform in-reach and engagement with members to inform about community services.
• Educate members on supported housing options and Medicaid benefits.
• Coordinate meetings with members and guardians to discuss housing interests.
• Document activities and maintain confidentiality per HIPAA and state laws.
