Job summary

Clarity Pediatrics, Inc. is seeking a Nurse Care Coordinator (RN) for a fully remote position in California.

You will collaborate with physicians, APPs, psychologists, and therapists to coordinate pediatric care for chronic conditions, manage appointments, educate families, and handle medication authorizations. Responsibilities include care scheduling, referrals, patient education, lab coordination, documentation across EHRs, and team collaboration.

Qualifications

• Active California RN license in good standing.
• Proficient with multiple technology platforms and EHR systems.
• Strong organizational, communication, and interpersonal skills.
• Ability to manage competing priorities and work remotely.
• Comfort serving diverse, pediatric populations with complex needs.
• Startup mindset with willingness to iterate on care models.
• Bilingual English/Spanish is a plus (preferred).

Responsibilities

• Coordinate, schedule, and reschedule clinical appointments across the care team.
• Manage referrals to specialists and external clinical services.
• Provide patient and family education on chronic condition management and treatment plans.
• Process prior authorization requests and coordinate with pharmacies.
• Document clinical interactions accurately in EHR and participate in team meetings.
• Monitor patient progress and address care gaps with clinical supervisors.
