Job summary

Clarity Pediatrics is seeking a remote Nurse Care Coordinator (RN) to join our interdisciplinary team. You will coordinate clinical appointments, educate families, manage medication authorizations, and serve as the primary contact between families, providers, and pharmacies.

Requirements include an active California RN license, strong EHR skills, and the ability to work remotely. Preferred bilingual English/Spanish.

Qualifications

• Active California RN license required (Associate's, Bachelor's, or Master's accepted).
• Proficiency with multiple technology platforms and EHR systems.

Responsibilities

• Coordinate, schedule, and reschedule clinical appointments across the care team.
• Manage referrals to specialists and external services.
• Provide patient/family education and support medication authorizations.
• Communicate updates to referring physicians and document in EHR.
