Qualifications

Responsibilities

• Provide remote supportive care for patients requiring additional monitoring or assistance.
• Promote quality patient care, comfort, and a safe environment in collaboration with the Registered Nurse.
• Interact with co-workers, visitors, and staff in alignment with Jefferson values.
• Recognize patients and family/designated care partners while providing respectful, compassionate, coordinated care.
• Communicate with and support the interprofessional team, patient, family, and designated care partners.
• Observe assigned patients for safety.
• Report changes in patient condition or behavior to professional staff.
• Complete required reports and documentation.
