Job summary

University of Miami is seeking a full-time Patient Navigator to work remotely out of the Medley, FL office. You will help patients overcome barriers to care, coordinate treatment plans, and monitor follow-up with caregivers.

This role emphasizes access to insurance, social services, and seamless communication with providers. Responsibilities include evaluating patients, guiding insurance verification, coordinating appointments, and educating patients on health resources.

Qualifications

• High School diploma or equivalent required.
• Minimum 3 years of relevant work experience required.
• Ability to process and handle confidential information with discretion.
• Ability to communicate effectively in both oral and written form.
• Ability to handle difficult and stressful situations with professional composure.
• Ability to maintain effective interpersonal relationships.
• Ability to understand and follow instructions.
• Building partnership with local community physicians and referral sources to the UHealth Systems.
• Maintaining communication with patients and healthcare providers to monitor patient satisfaction.
• Arranging special needs such as transportation, interpreting services, etc. as needed for appointments.
• Facilitate financial support, paperwork or medical records linkage to follow-up services.

Responsibilities

• Conducts patient evaluations to develop effective treatment plans.
• Ensures patients have access to insurance and social services for which they are eligible.
• Provides reminders of appointments and liaises between patients and providers.
• Identifies and resolves barriers to care.
• Educates and counsels patients on health issues and life situations.
• Maintains knowledge of community health resources and programs.
• Pre-registers and coordinates with insurance verification teams.
• Verify enrollment and deductible status for special contract programs.
• Notify parties of appointment times, referral criteria and insurance details.
• Intervene as liaison/advocate for patients, physicians, and staff.
