Job summary

Wider Circle in Indianapolis, IN is seeking a Part-Time Community Engagement Lead to run Connect for Life, a program that connects neighbors for better health and shared wellbeing. You will build trust with members through group and one-on-one interactions, host events, and support engagement to improve health outcomes.

You will conduct home visits, outreach by phone, and coordinate with health plans and providers to connect members to care resources.

Qualifications

• 3+ years in community outreach, teaching, or facilitation.
• Associate's degree preferred; 5+ years of relevant experience may substitute.

Responsibilities

• Lead the Connect For Life program, building trust and promoting health.
• Plan, host virtual and in-person events and coordinate outreach.
• Proactively contact members, conduct home visits, and monthly engagement.
• Document interactions and track engagement in Wider Circle systems.
• Identify natural leaders to serve as Wider Circle Ambassadors.
• Develop personalized plans to enroll prospective members.
