Responsibilities

• Patient Care: Provide primarily remote care to palliative care patients, responding to triage needs and more urgent concerns through video visits.
• Patient Satisfaction: Communicate with patients and families with empathy and understanding, providing education, support, and guidance related to their care.
• Care Coordination: Collaborate with the palliative care team to assess patient needs, provide appropriate recommendations, and coordinate care.
• In-Person Coverage: Complete required in-person training and provide occasional in-person coverage for vacation and sick-call needs.
