Job summary

HealthPartners is seeking a Licensed Practical Nurse to join the Palliative Care team in a remote, telephonic capacity. The role supports clinicians and other care professionals in delivering compassionate care to patients and families in home or community settings.

Ideal candidates have MN LPN licensure, BLS, and likely experience in hospice/home care or palliative settings, with strong communication and teamwork skills. M-F 8am-5pm schedule is typical.

Qualifications

• Graduation from a state approved school of practical nursing.
• Current MN LPN license in good standing.
• BLS certification for health professionals maintained.
• Ability to communicate clearly and work in a team.

Responsibilities

• Provide nursing care to patients in home or community settings.
• Support clinicians, social workers, and chaplains with patient care.
• Maintain professional communication and handle time-sensitive tasks remotely.
