Job summary

Elevance Health is seeking an Outreach Care Specialist for a virtual, full-time role with occasional required in-person training sessions. The position supports Rural Health Transformation Grant initiatives and focuses on family-centered care coordination across diverse community settings.

Responsibilities include coordinating follow-up care, assessing adherence to treatment plans, and promoting culturally inclusive engagement.

Qualifications

• Requires a HS diploma or equivalent and a minimum of 1 year related experience or an equivalent background.
• Certified nurse assistant (CNA) or certified medical assistant (CMA) and/or BS/BA degree preferred.
• Bilingual candidates preferred.
• Strong oral, written and interpersonal communication skills.

Responsibilities

• Coordinate follow-up care plans by scheduling appointments or enrolling members in programs.
• Assess member adherence to medical treatment plans via phone or on-site visits.
• Identify barriers to plan compliance and coordinate resolutions.
• Identify opportunities that impact quality goals and recommend process improvements.
• Coordinate referrals to local, state, or federally funded programs.
• Coach members on ways to reduce health risks and improve outcomes.
