Job summary

CareBridge Health, part of the Elevance Health family, is seeking an Outreach Care Specialist. This role supports member care plans, coordinates follow-up services, and aims to improve health outcomes in a flexible, mostly virtual setting.

The position requires a HS diploma with at least 1 year of related experience; CNA/CMA or BS/BA are preferred. Proficiency in Microsoft Office and bilingual abilities are a plus. Hybrid/virtual work arrangement applies, with in-person training as needed.

Qualifications

• HS diploma or equivalent; at least 1 year related experience.
• Preferred CNA or CMA certification; BS/BA degree preferred.
• Bilingual candidates preferred.
• Proficiency in Microsoft Office.

Responsibilities

• Coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs.
• Assesses member compliance with medical treatment plans via telephone or on-site visits.
• Identifies barriers to plan compliance and coordinates resolutions.
• Identifies opportunities that impact quality goals and recommends process improvements.
• Recommends treatment plan modifications and determines need for additional services with case management and provider.
• Coordinates identification of and referral to local, state or federally funded programs.
• Coaches members on ways to reduce health risks.
• Prepares reports to document case and compliance updates.
• Establishes and maintains relationships with agencies identified in appropriate contract.
