Location: Virtual: This role enables associates to workvirtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.Alternate locations may be considered if candidatesresidewithin a commuting distance from an office
This position will have a schedule of 8:30am-5pm EST.

The Outreach Care Specialist supports various initiatives related to the Rural Health Transformation Grant.
Promotes coordination and integration of family-centered care to ensure participation and involvement of family and/or caregivers.
Supports the development of family/caregiver Project ECHO sessions as it relates to identified needs.
This role will help ensure identification and outreach to traditional and nontraditional community settings where families gather.
This role will promote culturally responsive and inclusive engagement strategies as well as development of trusted connections to increase family participation and access to services.
Responsible for ensuring that appropriate member treatment plans are followed on less complex cases and for proactively identifying ways to improve the health of our members and meet quality goals.
