Job summary

Affect is seeking an empathic Peer Support Specialist and Case Manager certified in Oregon to join our team. You will provide peer support and case management for members with substance use disorders, working with the clinical team to deliver non-clinical support, guidance, mentoring, and resources.

Your lived recovery experience is a strength at Affect, helping build trust and resilience. This fully remote role requires Oregon residency and an Oregon OHA certification.

Qualifications

• Education: High school diploma or equivalent required.
• Certification: Active Oregon OHA Peer Support Specialist.
• Experience: At least 2 years as a peer support specialist preferred.

Responsibilities

• Provide peer support to members through one-on-one and group sessions, to foster community, build self-esteem, and support long-term recovery.
• Provide member case management support with accessing community resources and overcoming barriers to treatment.
• Perform telephone outreach to potential members referred to Affect from referral partners.
• Provide education on wellness and recovery skills, while serving as a mentor and role model.
• Maintain timely and accurate documentation in our EMR.
