Job summary

UnitedHealth Group's Amedisys Hospice team is seeking an experienced Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care across remote and field settings.

You will assess patient conditions, escalate when needed, educate patients and caregivers, and maintain accurate documentation while working with interdisciplinary teams.

Qualifications

• Current RN license in applicable state(s).
• Current CPR certification.
• Minimum 1 year of nursing experience, hospice/home health, or acute care.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone or in-person visits and escalate as needed.
• Coordinate with physicians, care teams, and caregivers for timely care.
• Educate patients and caregivers on symptom management and care plans.
• Maintain accurate documentation of all interactions and decisions.
