UnitedHealth Group is seeking a Weekend On-Call Hospice RN to provide after-hours clinical support, triage patient needs, and coordinate care across remote and field settings. This role ensures continuity and quality outcomes while supporting interdisciplinary teams.

Qualifications include an active RN license in the assigned state, 1+ year nursing experience (hospice/home health preferred), CPR certification, and driver's license with reliable transportation.

• Weekend On Call RN 5pm Friday - Monday 8am

As the Hospice RN On-Call, you are responsible for providing after-hours clinical support, triaging patient needs, and coordinating care to ensure continuity and quality outcomes. This role delivers remote and field-based nursing support, assesses patient conditions, and collaborates with interdisciplinary teams to address urgent and emergent patient needs.

Primary Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and **escalate** care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions

Required Qualifications

• Current RN license, specific to the state(s) you are assigned to work
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification
• Valid driver's license, reliable transportation, and liability insurance

Preferred Qualifications

• CNA experience within the last two years

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $75,620 to $113,430 annually based on full-time employment. We comply with all minimum wage laws as applicable.
