Job summary

Form Health, a virtual obesity medicine clinic, seeks ABOM-certified physicians for a fully remote, US-based part-time role. You must hold an MD/DO, ABOM certification, and an active US state license, with a minimum of 20 hours per week (16 clinical, 4 admin).

You will deliver evidence-based obesity care, collaborate with APPs and registered dietitians, manage pharmacotherapy, ensure consistent documentation, and contribute to quality improvement in telemedicine.

Qualifications

• Must have MD or DO degree.
• ABOM board certification is required.
• Active US medical license with eligibility for additional states.
• Minimum 2 years as attending physician or fellowship in Obesity Medicine.
• Experience mentoring APPs and collaborating with dietitians is preferred.

Responsibilities

• Provide direct patient care focusing on obesity treatment.
• Review charts and conduct visits to ensure quality care.
• Develop treatment plans and manage pharmacotherapy for obesity.
• Collaborate with APPs and Registered Dietitians in care delivery.
• Maintain clinical documentation and adhere to care pathways.
• Participate in quality improvement and departmental meetings.
• Support telemedicine operations and patient-centered care.
