Job summary

Form Health is seeking ABOM-certified physicians specializing in obesity medicine to join a remote, physician-led care team. The role emphasizes collaborative care with APPs and Registered Dietitians, focusing on evidence-based weight management and pharmacotherapy.

Licensure in any US state is required, with flexibility for additional state licenses as needed. The position is fully remote (US-based) with options for 32, 36, or 40 hours per week, combining clinical care and administrative time.

Qualifications

• MD or DO with active medical license in the US.
• ABOM board certification required.
• 2+ years as attending physician or completed obesity medicine fellowship.
• Experience with obesity pharmacotherapy and chronic weight management.
• Mentoring APPs and collaboration with dietitians in obesity care.
• Telehealth patient care experience is preferred.

Responsibilities

• Provide direct patient care focusing on obesity treatment via telemedicine.
• Review charts and manage care plans for complex cases.
• Collaborate with APPs and Registered Dietitians on patient care.
• Prescribe non-controlled obesity medications for patients.
• Ensure documentation and treatment pathways are followed within the team.
• Participate in team quality improvement and departmental activities.
• Attend relevant meetings and contribute to program goals.
