Form Health is a virtual obesity medicine clinic delivering multidisciplinary, evidence-based care through telemedicine. We seek ABOM-certified physicians with MD/DO credentials to lead patient care and collaborate with APPs and dietitians in a remote, flexible W-2 role.

The ideal candidate maintains an active US medical license, has ABOM certification, and thrives in a collaborative, patient-centered team delivering high-quality care nationwide.

Qualifications

• MD or DO degree.
• ABOM board certification required.
• Active unrestricted medical license in any US state.

Responsibilities

• Provide direct patient care in obesity medicine via telemedicine.
• Review charts and conduct visits to ensure quality and continuity of care.
• Develop treatment plans and manage complex cases with APPs and dietitians.
