Job summary

Form Health seeks an experienced obesity medicine physician or endocrinologist for remote, telemedicine care. You will manage a dedicated patient panel focused on evidence-based weight management and collaborate with dietitians and other specialists.

The role offers a flexible schedule, with participation in an after-hours call rotation and protected time for administrative duties in a fully remote setting.

Qualifications

• MD or DO with ABOM diplomate or Endocrinology board certification.
• Licensed to practice in AL, FL, GA, IN, KY, MI, MS, NC, OH, SC, or TN.
• LOQ with IMLC (Interstate Medical Licensure Compact).
• Active multi-state licenses are preferred.

Responsibilities

• Provide direct obesity medicine telemedicine care with a dedicated patient panel.
• Collaborate with diabetes/APP teams and dietitians as part of a team approach.
• Participate in rotating after-hours call schedule with leadership.
• Prescribe non-controlled obesity medications under supervision.
• Ensure documentation and treatment pathways are consistently followed.
