Job summary

Form Health, a virtual obesity medicine clinic, is seeking obesity medicine physicians to provide longitudinal telehealth care and collaborate with care team. This is a remote, full-time position with flexible schedules.

You will lead a personal patient panel, work with dietitians, and participate in rotating after-hours call. Board certification in ABOM or Endocrinology is required, and LOQ with IMLC. Licensed in AR, IA, KS, MN, MO, NE, ND, OK, SD, or WI; bilingual Spanish preferred.

Qualifications

• MD or DO with ABOM diplomate or endocrinology board certification.
• Licensed to practice in AR, IA, KS, MN, MO, NE, ND, OK, SD, or WI.
• Must hold LOQ with IMLC.
• Experience credentialing with insurers and multiple state licenses preferred.
• Excellent interpersonal skills and commitment to evidence-based obesity care.
• Telemedicine visit experience and web-side manner.
• Bilingual Spanish preferred.

Responsibilities

• Provide direct patient care for obesity treatment via telemedicine.
• Manage own patient panel and collaborate with the dietitians.
• Lead treatment planning and medical management for complex cases.
• Prescribe non-controlled obesity medications.
• Ensure documentation and treatment pathways are followed.
• Support APP development and potential supervision.
• Participate in team-based care and quality improvement.
• Attend departmental meetings.
