Job summary

All Abilities, Inc., a Lifesteps subsidiary, seeks a full-time Nursing Home Transition Service Coordinator to assist participants in transitioning from nursing facilities to community living. Travel up to 50% weekly within 50 miles of Hazelton, PA is required.

The role emphasizes coordinating housing options, waivers, and supports while collaborating with nursing home staff and family members to ensure smooth transitions.

Qualifications

• High School Diploma or GED required.
• Strong communication and customer service skills.
• Ability to organize multiple tasks and manage time effectively.
• Valid driver’s license and willingness to travel up to 50% per week (within 50 miles of Hazelton, PA).
• Proficiency with Microsoft Office Suite, Outlook and Adobe PDF.

Responsibilities

• Assist participants with housing options and utility services.
• Coordinate with nursing home staff and family members as needed.
• Verify waivers or income funding; arrange community supports.
• Possibly conduct case management across specified PA counties.
