Job summary

CenterWell Senior Primary Care seeks a Nurse Practitioner (VSP) to apply advanced education and clinical skills in virtual visits, focusing on chronic condition management and preventive care for Humana members.

This remote, part-time APP role runs 20-30 hours per week for six months, reporting to the Associate Director, Advanced Practice Provider. Requires Master’s degree, ANCC or AANPCB certification, and an active NP license in multiple states.

Qualifications

• Masters degree or higher in nursing or related field.
• Active ANCC or AANPCB certification.
• Unrestricted NP license in at least two states among FL, TX, AZ, GA, LA, NC, SC, MS, VA.

Responsibilities

• Conduct virtual visits with CenterWell patients to manage chronic conditions.
• Address acute and chronic needs; document patient records in Athena.
• Verify benefits and coordinate care with PCP.
• Comprehensive evaluation including med history review and coordinating social needs.
• Follow up on results, meds, and PCP follow up.
