Job summary

Pear Suite is seeking a Nurse Practitioner to join as a PRN clinician in a fully remote, virtual care setting. You will conduct telehealth evaluations, coordinate with CHWs, and develop care plans focused on medical and social needs.

The role offers flexibility with shifts based on patient demand and a clear path to a more regular schedule as volume grows. The ideal candidate is an experienced NP with telehealth background, capable of documenting visits and navigating Medicaid/Medicare

Qualifications

• Active, unrestricted NP/APRN license required in practicing state(s).
• Experience in primary care or family medicine and telehealth is a plus.
• Spanish or other language fluency is preferred.
• Experience working with Medicaid/Medicare populations is desirable.

Responsibilities

• Conduct telehealth clinical evaluations for acute, chronic, preventive, and social needs.
• Refer for care coordination and CHW services when indicated.
• Develop individualized care plans addressing clinical and social determinants of health.
• Collaborate with Community Health Workers to support patient engagement and follow-through.
• Document visits accurately per requirements.
• Help patients navigate healthcare systems and community resources.
