Job summary

Savoy Life is seeking a motivated Nurse Practitioner to provide telemedicine support to senior living communities. In this fully remote role, you will collaborate with on-site RNs to develop care plans and conduct virtual visits for chronic condition management. Ideal candidates will possess an active NP license and experience in geriatric medicine. The position offers a competitive salary of $115,000 - $130,000 along with comprehensive health care and unlimited PTO.

Qualifications

• Must have strong experience in geriatric medicine and chronic disease management.
• Expertise in developing care plans for patients with multiple chronic conditions is essential.
• Ability to work autonomously in a remote setting and adapt to evolving workflows.

Responsibilities

• Conduct new patient and establish-care visits via telemedicine.
• Collaborate with on-site RNs to develop individualized care plans.
• Perform telemedicine priority care visits for common ailments.
• Partner with the Chief Clinical Officer to adjust workflows as needed.
• Assist in developing training materials for new clinicians.
